Question Listing for Seafarer's Identification & Record Book -(SIRB)

Question: How can I obtain a Seafarer's Identification & Record Book (SIRB)?
 
Answer: To receive a Liberian Seafarer's Identification & Documentation Book you must submit an Application for Seafarers Identification & Record Book; RLM-273/279, through any authorized Filing Agent which mans the Liberian vessel on which you will be sailing. This application must be accompanied by a copy of your national passport or similar identification document, and proof of seatime listing your position aboard the ships as well as the respective gross tonnage, horsepower (kilowatts), and types of ships. A copy of a physical examination completed within the past twelve months must also accompany the application along with two passport size photos. The requisite fees for Books are listed in Annex 5 of the RLM-118, "Requirements for Merchant Marine Personnel".
